Finding the Pandemonium Threshold is less about raw power and more about following the seasonal trail in the right order. You can be deep into Torment, carrying D4 Gold for upgrades and still see no entrance near Dannica in Zarbinzet. That usually means one quest step was skipped. The Threshold is tied to the Death Cult storyline, and the game expects you to finish a specific Realmwalker route before the Corrupted Reaper becomes available.
Normal Pandemonium Ruptures are useful for seasonal progress, but they won't move this part of the quest forward. You need a Surging Rupture or the Colossal Rupture in the Fields of Desecration, southeast of Zarbinzet. The Colossal version is usually the easier choice. It reliably leads to a Realmwalker, so you're not stuck circling Helltides hoping the right event turns up.
Once that first encounter is done, the loop becomes much simpler. Deathtoll Chambers can provide Superior Lair Keys, while repeatable Glints of Hope rewards can add more key support and Pandemonium Fragments. Those keys are for the reward cache after beating the Corrupted Reaper in Torment I or above. They do not unlock the first entrance. It's a small distinction, though it saves a surprising amount of wasted time.
Pandemonium Fragments are used in the Horadric Cube's Mythic upgrade recipe, which now costs four fragments. Pick the equipment slot first. The Unique item you put in decides the slot of the Mythic result, not the exact item you'll receive. Don't throw in a piece your build still needs just because it has item power 850 or higher. Hold fragments until you've got a weak slot and several useful Mythic outcomes available.
